Job summary

Complete Home Care - Private Duty LLC seeks a dedicated Home Health Aide to assist clients at home with basic health maintenance tasks according to a Plan of Care.

You will help with transfers, mobility, feeding and exercises, and will communicate with staff about changes in patient condition and care needs. Documentation of care is required, with a focus on safety and infection control.

Qualifications

• 1 year experience as a home health aide or CNA in home health setting, or completion of formal nursing arts training as allowed by state law.
• Ability to establish and maintain effective working relationships with staff and leadership.
• Shall be able to read, write and comprehend English.

Responsibilities

• Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).
• Communicate effectively with administrative staff regarding client/family needs assessments, including reporting significant changes in client condition.
• Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.
• Contribute to continuity of care, effective communication between all care team members, and participate in Case conferences/Team Meetings.
• Apply infection control and universal precautions in coordination/performing client care activities to protect staff and patients.
• Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency; completes training as assigned.
• Meet mandatory continuing education requirements of the Agency/licensing board.

Assists clients, in their place of residence, with personal healthcare and health maintenance activities as assigned. Includes, but is not limited to, transferring, health maintenance activities, feeding, mobility, and exercises.

Patient Population

Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.

Essential Functions

Requirements

Experience

• (1) year experience as a home health aide or certified nursing assistant in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Skills

• Ability to establish and maintain effective working relationships with all segments of the staff and the Agency leadership team.
• Shall be able to read, write and comprehend English.

Education

• High School Graduate or equivalent

Licensure/Certification

• Home Health Aide Certificate or other similar certification held in good standing. Certification requirements may vary by state and/or participating program.
• A formal certificate of completion for an HHA/CNA training program that meets or exceeds standards established by Health and Human Services. Completion of all in-service hours per year. Ongoing requirements may vary by state and/or participating program.

Current CPR certification

• Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
• Reliable transportation and auto liability insurance.
• On-going employment as an HHA always requires the maintenance of a valid HHA certificate in the state where the caregiver practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.

Physical Requirements

• Prolonged standing/walking is required.
• The ability to lift/transfer up to 50 lbs.
• Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
• Meet patient/family individualized psychosocial needs.
• Requires hand-eye coordination and manual dexterity.

Environmental/Working Conditions

• Works in patients’ homes in various conditions.
• Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
• Some exposure to unpleasant weather; PRN emergency call.
• Ability to travel locally and work a flexible schedule.
