Job summary

BrightStar Care of Denton is seeking a Primary Registered Nurse to care for our patients in home health and facility settings. You will educate patients and families on prevention and healthy habits while delivering high-quality nursing care with limited supervision.

The ideal candidate is an experienced, compassionate RN with strong assessment skills, excellent communication, and the ability to coordinate care across teams.

Qualifications

• Current unrestricted license as a registered nurse in Texas.
• Bachelor’s degree in Nursing preferred.
• Wound care experience preferred.
• Proficiency in IV skills and Central Line (PICC/PORT) care required.
• Certified CPR and TB test or chest x-ray.
• Minimum one year of experience in home health, primary care, or health facility; three to five years preferred.
• Possess exceptional nursing assessment skills.
• Licensed driver with valid automobile insurance.
• Ability to work weekends, nights and holidays.
• Must be able to lift up to 50 pounds.

Responsibilities

• Must perform health care responsibilities and monitor health conditions.
• Conduct comprehensive nursing assessments annually.
• Coordinate and ensure quality nursing care and treatment plans (BSP).
• Discuss assessments at IDT meetings.
• Assist in developing the IHP.
• Administer or delegate medication as appropriate.
• Monitor medication use and treatment progress.
• Ensure unlicensed staff perform appropriate tasks.
• Assist individual in obtaining routine and emergency medical services.
• Makes referrals for appropriate medical services.
• Perform procedures prescribed by a physician or as per nursing standards.
• Available for up to 40 hours per week during normal business hours.

Responsibilities

• Must perform health care responsibilities and monitor the Individual’s health conditions;
• Conducts the comprehensive nursing assessments to evaluate health and functional needs for the Individual at least annually;
• Responsible for overall coordination, compliance, quality of nursing care provided to the individual, and compliance with treatment plans (BSP);
• Discusses the assessment as part of the (IDT) Inter disciplinary Team meeting;
• Assists in the development of the (IHP) Individual Habilitation Plan;
• Administers medication or delegates medication administration as appropriate;
• Monitors the Individual’s health status, use of medications, and treatment progress;
• Monitor’s health risks, data, and information, including ensuring that an unlicensed Staff Member is performing only those nursing tasks identified from a nursing assessment;
• Assists the Individual to secure routine and emergency medical Services;
• Makes referrals for appropriate medical Services;
• Performs health care procedures ordered or prescribed by a physician or medical practitioner and required by standards of professional practice or law to be performed by an RN or LVN; and
• Must be available for up to 40 hours per week during normal business hours.

Requirements

• Current unrestricted license as a registered nurse in the state of Texas
• Preferred: Bachelor’s degree in Nursing.
• Wound care experience is preferred
• Proficiency in IV Skills and Central Line (PICC/PORT) care is required
• Certified CPR and negative TB skin test or chest x-ray
• Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred. Three to five years is preferred
• Possess exceptional nursing assessment skills
• Analytic/Critical thinker.
• Licensed driver with valid automobile insurance
• Ability to work on weekends, nights and holidays
• Reliable transportation
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
