Job summary

Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency.
• Current, full, active, and unrestricted registration as a graduate professional nurse in the United States or District of Columbia.
• State licensure required; may include GNT pathway and bridge programs as described by VA qualification standards.

Responsibilities

• Assumes responsibility for coordination of care focused on patient education, self-management, and patient satisfaction.
• Administers medications and procedures per established policies and guidelines.
• Collaborates with interdisciplinary team to influence care outcomes and implement evidence-based practice.
• Develops, implements, evaluates, and revises policies and procedures as part of care delivery.
• Provides health promotion, disease prevention, and chronic care management.
• Fosters professional development and supports colleagues in primary care settings.
• Ensures evaluation and access for patient panels using registries and care coordination.
