Job summary

Veterans Health Administration in Fresno seeks a Staff Registered Nurse in Primary Care to apply the nursing process and evidence-based practice while collaborating with Veterans, providers, clerical/clinical staff, and caregivers to develop a patient-driven care plan.

The RN coordinates care with internal and external VA services to facilitate patient transitions and meet patient needs. Duties include education, medication administration, and care coordination within a multidisciplinary team.

Qualifications

• Must be a Graduate of a school of professional nursing approved by accrediting agency (ACEN/CCNE)
• Current, full, active, and unrestricted registered nurse license in the United States
• Foreign-educated nurses must meet equivalency requirements and licensure
• State licensure required for appointment
• GNT exception allows temporary appointment while pursuing licensure

Responsibilities

• Coordinates care focusing on patient education, self-management, and patient satisfaction throughout the continuum of care
• Administers medications and procedures per policies/guidelines
• Collaborates with interdisciplinary team to improve care outcomes
• Contributes to development, implementation, evaluation of policies and procedures
• Ensures appropriate evaluation and access to patient panels
• Supports professional development and safe nursing care in a collaborative environment
• Evaluates outpatient outcomes of evidence-based decisions in primary care
