Qualifications

• RN with a baccalaureate nursing degree and appropriate state licensure.
• One year RN experience in applicable care setting.
• Strong communication and teamwork.
• Computer skills; EMR experience preferred.

Responsibilities

• Coordinate nursing care and patient/family education in Primary Care.
• Provide direct patient care and transitions of care.
• Collaborate with providers and Care Team members.
• Lead through preceptorship and quality improvement activities.
