Job summary

Qualifications

• English proficiency required to serve in direct patient care.
• Graduate of a professional nursing program approved by ACEN or CCNE.
• Current, full, active, and unrestricted RN license in a US state/territory (or DC).

Responsibilities

• Demonstrates clinical knowledge in assessing, planning, implementing, documenting and evaluating care for a designated group of patients across the continuum of care.
• Triage and use a collaborative team approach to identify, analyze and resolve patient care problems.
• Collaborates with patients to assess needs, goals, and resources for discharge, recovery, and wellness.
• Maintains knowledge of multidisciplinary resources and refers patients to community services as appropriate.
• Advances care through problem-solving and evidence-based practice to improve system/processes.
• Navigates health systems and intervenes to achieve patient outcomes.
