• Use EMR system with training; documentation handled by scribes.
• Rotated call schedule with 100% remote support and call center assistance.

The position offers a collaborative team environment with extensive support and resources.

Responsibilities

A nationwide primary care organization, WCS Healthcare Partners, is seeking an Advanced Practice Provider for their Hartford County location. This role focuses on providing care to the geriatric population, ensuring high-quality, value-based care that aims to reduce hospitalizations for Medicare patients. The position offers a collaborative team environment with extensive support and resources.

• Provide primary care services to a patient load of 12-16 patients per day, primarily Medicare recipients aged over 65, with some Medicaid patients.
• Work collaboratively with a care team consisting of 1 MD, an APP, a scribe, a medical assistant, and shared RN/social work support.
• Utilize specialty support through a partnership with a telemedicine company for quick consults in areas such as dermatology, cardiology, and gastroenterology.
• Use an innovative EMR system with training provided; documentation handled by scribes.
• Participate in a rotated call schedule, averaging 1 week every 3 months, with 100% remote support and call center assistance.

This is a full-time permanent position with a 4-6 week orientation and training period.
