Job summary

Qualifications

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA
• 1 year of experience in a primary care or outpatient setting

Responsibilities

• Provide high-quality primary care services to adult patients (18+) in an outpatient setting
• Develop and manage personalized care plans for patients
• Collaborate with a multidisciplinary team to improve patient outcomes
• Maintain accurate and timely documentation of patient encounters and treatment plans
