Job summary

WhiteCap Search is seeking a Spanish-speaking Primary Care Advanced Practice Provider for our Brooklyn, NY location. This full-time, permanent role focuses on geriatric care within a nationwide primary care organization.

You will work with a physician, another AP, a scribe, and RN/social work support, with remote call coverage and EPIC EMR in use. A comprehensive benefits package is offered.

Qualifications

• Board certification required.
• NY state license required.
• Training as a Family NP or Adult NP.
• Previous primary care or SNF experience preferred, but not required.

Responsibilities

• Provide primary care services to a geriatric patient population, primarily Medicare recipients.
• Collaborate with a care team including physician, APN, scribe, MA, and RN/social work support.
• Utilize telemedicine for specialist consults (dermatology, cardiology, gastroenterology).
• Use EPIC EMR with scribe support to manage records and care plans.
• Participate in a call schedule with limited, 100% remote coverage.

Job description

Join a nation-wide primary care organization focused on providing exceptional care to the geriatric population. We are seeking a Spanish-speaking Primary Care Advanced Practice Provider for our Brooklyn location. This is a full-time, permanent position with a comprehensive benefits package.

Responsibilities

• Provide primary care services to a patient population primarily consisting of Medicare recipients (over age 65), with some Medicaid patients under age 65.
• Work collaboratively with a care team consisting of a physician, advanced practice provider, scribe, medical assistant, and shared RN/social work support.
• Utilize specialty support from a partnered telemedicine company for quick consults with specialists such as dermatologists, cardiologists, and gastroenterologists.
• Utilize EPIC EMR, with scribe support, to manage patient records and care plans.
• Participate in a call schedule rotated amongst the market, with calls being limited and 100% remote.

Schedule

• Monday to Friday, 8am-5pm
• No nights or weekends
• 12-16 patients per day once fully ramped up
