Job summary

WCS Healthcare Partners in Hartford County seeks an Advanced Practice Provider to deliver geriatric primary care focused on value-based outcomes for Medicare patients. You will manage a panel with 12-16 patients daily in a collaborative team with an MD, another NP, a scribe, medical assistant and RN/social work support.

A telemedicine partner provides quick specialty consults; EMR training is provided and documentation is handled by scribes.

Qualifications

• Board-certified NP with FNP or Adult NP training.
• Active CT state license required.
• Experience in primary care or SNF preferred.

Responsibilities

• Provide primary care to 12-16 patients daily, mainly Medicare beneficiaries over 65.
• Collaborate with a care team including MD, NP, scribe, MA, and RN/SW support.
• Utilize telemedicine for quick consults in dermatology, cardiology, and gastroenterology.
• Use an EMR system with training; documentation handled by scribes; rotated on-call schedule with remote support.

Job description

A nationwide primary care organization, WCS Healthcare Partners, is seeking an Advanced Practice Provider for their Hartford County location. This role focuses on providing care to the geriatric population, ensuring high-quality, value-based care that aims to reduce hospitalizations for Medicare patients. The position offers a collaborative team environment with extensive support and resources.

Responsibilities

• Provide primary care services to a patient load of 12-16 patients per day, primarily Medicare recipients aged over 65, with some Medicaid patients.


• Work collaboratively with a care team consisting of 1 MD, an APP, a scribe, a medical assistant, and shared RN/social work support.


• Utilize specialty support through a partnership with a telemedicine company for quick consults in areas such as dermatology, cardiology, and gastroenterology.


• Use an innovative EMR system with training provided; documentation handled by scribes.


• Participate in a rotated call schedule, averaging 1 week every 3 months, with 100% remote support and call center assistance.
