Job summary

Fast Pace Health is seeking an experienced Primary Care Provider to deliver patient-centered care in a rural community setting. The role emphasizes evidence-based, team-based practice with a focus on preventive care, chronic condition management, and wellness visits.

The ideal candidate will have NP or PA credentials with primary care experience, be able to work in a fast-paced environment, and engage in continuous quality improvement while supporting patients through multiple care channels.

Qualifications

• NP/PA with 1 year in direct primary care.
• Proficient in chronic care management and preventive care.
• Experience with pediatric exams (3+ years old) is preferred but not required.

Responsibilities

• Deliver high-quality primary care in a Team-based setting.
• Provide annual wellness visits and preventive screenings.
• Order and interpret diagnostic tests to guide treatment.
• Collaborate with physicians and other clinicians on care plans.
• Prescribe medications and provide patient education.
