Qualifications

Responsibilities

• Develop personal relationships with patients.
• Monitor and adapt individualized health programs over time.
• Educate patients in health management and disease prevention.
• Assume responsibility for day-to-day supervision of PAs/NPs.
• Promote continuity of patient care by collaborating with the healthcare team.
