TGH Senior Center Powered by Greenbrook Medical is seeking a Primary Care Physician to deliver exceptional care to elderly patients. The role involves managing a limited patient panel, facilitating thorough patient assessments and personalized care plans, and collaborating closely with a healthcare team to enhance patient outcomes.
The ideal candidate will have 2-6 years of experience in primary care and a strong dedication to geriatric healthcare. Competitive compensation and benefits package included.
At Greenbrook Medical, we believe seniors deserve more from the healthcare system—more time, more care, more coordination, and more heart. We provide high-touch, relationship-based primary care to seniors, built around one simple idea: deliver the kind of care we’d want for our own parents.
Primary Care Physicians provide daily care to patient panels capped at approximately 500 patients. This model emphasizes work‑life balance while delivering a concierge‑like experience for patients. With a panel size limited to 500, physicians typically see around 12 patients per full day, allowing 30 to 45 minute visits to address each patient's needs thoroughly.
Patient appointments are generally scheduled between 9:00 a.m. and 3:30 p.m., leaving additional time for urgent patient needs and administrative tasks. Administrative burdens are significantly reduced thanks to a dedicated team of coders who handle pre‑charting and post‑visit reviews, streamlining clinical documentation. Medical assistants manage administrative and care coordination tasks, enabling physicians to focus entirely on patient care and providing an exceptional experience.
Primary Care Physicians will provide comprehensive, patient‑centered care to geriatric patients within a managed care, value‑based care model, ensuring optimal health outcomes and a high‑quality care experience.
At Greenbrook Medical, we believe the only way we accomplish our mission is by building the best team in healthcare. We do this through a culture of respect and belonging, ensuring our teammates feel cared for first and foremost.
We will extend equal employment opportunity to all applicants without regard to age, race, ethnicity, sex, religion, sexual orientation, gender identity, socioeconomic background, disability status, military affiliation, pregnancy or any other status protected under federal, state and local laws. We encourage all who share our mission to apply. Greenbrook Medical will provide reasonable accommodations during the recruitment process.
