AdvantageCare Physicians (ACPNY) is seeking an Internal or Family Medicine trained physician to join our primary care practice serving half a million patients across the New York metropolitan area. With more than 30 offices across NYC and Long Island, ACPNY is expanding our practice and enhancing services for communities throughout New York.

Qualifications

• MD/DO or foreign equivalent required.
• Three-year residency in Internal Medicine or Family Medicine (U.S. accredited).
• Board Eligible/Board Certified in Internal Medicine or Family Medicine.
• Current, unrestricted New York State license.
• Active DEA registration or eligible.
• Active Medicare/Medicaid Enrollment or eligible to enroll; BLS/ACLS required.
• Strong communication skills; comfortable with EMR and Office software.

Responsibilities

• Review patient history and perform physical examinations; diagnose and treat; refer to specialists as needed.
• Provide quality clinical diagnostic skills and review test results to recommend management including preventive care.
• Prescribe medications and formulate ongoing disease management plans.
• Deliver relationship-centered care addressing psychosocial and physical needs.
• Document patient information and maintain complete medical records.
• Communicate effectively with the clinical team to ensure total quality care.
• Participate in medical staff educational programs and meetings.
