North Country Healthcare is seeking a compassionate Primary Care Physician (Family Medicine or Internal Medicine) to join Weeks Medical Center in Whitefield, NH. The role blends outpatient care with resident education, about 60% clinic and 40% teaching duties, contributing to the White Mountains Medical Education Consortium (WMMEC).

You will build your patient panel while mentoring residents, shaping a rural residency program focused on high-quality, patient-centered care and meaningful

Qualifications

• Graduate from an accredited Medical College with a Medical Degree.
• Board-certified in Family Medicine or Internal Medicine after ACGME residency.
• NH medical license or ability to obtain.
• BLS from the American Heart Association.

Responsabilités

• Provide comprehensive primary care, including exams, diagnosis, treatment, and personalized care planning.
• Manage both acute concerns and chronic conditions with appropriate follow-up and monitoring.
• Deliver preventive care such as immunizations, health screenings, and annual wellness visits.
• Order and interpret diagnostic testing; follow up on results and adjust care plans as needed.
• Prescribe medications and treatments in alignment with evidence-based best practices.
• Educate patients and families on disease prevention, lifestyle changes, and long-term health management.
• Coordinate care with specialists and other providers to ensure seamless patient care.
• Collaborate with nursing and support staff to promote continuity, quality, and patient-centered care.
• Maintain timely, accurate documentation in accordance with Medical Staff Bylaws and policies.
• Participate in quality improvement initiatives and clinic call rotation.
