Qualifications

Responsibilities

• Provide clinical leadership across community, home, hospital, and skilled nursing settings.
• Collaborate with interdisciplinary team to develop and implement patient-centered care plans.
• Influence clinical standards, performance metrics, and care outcomes.
• Mentor Nurse Practitioners and other IDT members.
• Lead efforts to optimize patient outcomes and cost-effective care.

QUALIFICATIONS

• Completion of a physician residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
• Master's degree or doctoral degree, as applicable to the physician's professional credentials.
• Board certification in Internal Medicine, Family Medicine, or Emergency Medicine.
• Geriatric medicine experience, training, or certification is strongly preferred.
• Active, unrestricted physician license in the applicable state.
• Current state Controlled Dangerous Substances (CDS) certification, as applicable.
• Current DEA registration
• Eligible for or currently enrolled as a provider with Medicare and Medi-Cal
• Demonstrated knowledge of chronic disease management and geriatric medicine preferred.
• Ability to work collaboratively within an interdisciplinary care model.
• Willingness and ability to participate in a rotating night and weekend on-call schedule.
• Ability to provide care in multiple settings, including the PACE center, participant homes, hospitals, and nursing facilities.
