Job summary

Humboldt Park Health in Chicago seeks a family medicine physician to deliver comprehensive primary care in both inpatient and outpatient settings. You will perform thorough assessments, diagnose a range of conditions, and develop personalized treatment plans while documenting encounters in Epic and ensuring HIPAA compliance.

Collaborating with specialists, you will provide education on health maintenance and disease prevention, and participate in clinical research or quality improvement projects

Qualifications

• MD or DO with board certification in Family Medicine.
• Active, unrestricted Illinois medical license or eligibility.
• One year of attending or inpatient clinical experience.
• Proficiency with EMR/EHR systems (Epic) for documentation.
• Ability to collaborate across disciplines and educate patients.

Responsibilities

• Provide comprehensive primary care with inpatient and outpatient focus.
• Conduct medical assessments, diagnose acute and chronic conditions.
• Develop personalized treatment plans including medication management.
• Document encounters in EMR/EHR with HIPAA compliance.
• Coordinate with specialists to ensure integrated patient care.
• Educate patients on health maintenance and disease prevention.
• Participate in clinical research or quality improvement activities.
