You will diagnose, treat, and educate patients, collaborate with a multidisciplinary team, and participate in quality improvement initiatives. License eligibility in Illinois and ongoing professional development are required.

Qualifications

• MD or DO with Board Certification in Family Medicine.
• Current Illinois medical license or eligibility.
• Minimum 1 year of Attending or inpatient clinical experience.

Responsibilities

• Conduct thorough medical assessments and histories to diagnose acute and chronic conditions.
• Develop and implement personalized treatment plans including medication management.
• Document encounters accurately in EMR/EHR systems with HIPAA compliance.
• Coordinate with specialists to ensure comprehensive patient care.
• Perform routine procedures and educate patients on preventive care.
