Job description

Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Chicagoland clinics within our Healthcare & Medical Services department. As a Primary Care Physician at Oak Street Health, you will provide comprehensive, longitudinal care to a predominantly Medicare patient population, focusing on prevention, chronic disease management, and improving health outcomes in underserved communities. You will manage a panel of adult patients, perform thorough histories and physical exams, diagnose and treat acute and chronic conditions, and coordinate care across a multidisciplinary team including nurses, medical assistants, care managers, social workers, and behavioral health professionals. You will use evidence-based guidelines, clinical decision support tools, and population health data to proactively identify care gaps, reduce avoidable hospitalizations, and optimize quality metrics.

In this role, you will build strong, trusting relationships with patients and their families by providing patient-centered, culturally sensitive care, engaging in shared decision-making, and dedicating adequate time to each visit. You will leverage an electronic health record to document visits, track test results, manage referrals, and communicate with patients and the care team. You will participate in care team huddles, quality improvement initiatives, and case conferences to ensure coordinated, holistic care. You will also contribute to a collaborative and inclusive culture by mentoring team members, sharing best practices, and actively participating in clinical meetings and continuous learning opportunities.

The ideal candidate is board certified or board eligible in Internal Medicine or Family Medicine, comfortable managing complex geriatric and multi-morbid patients, and enthusiastic about working in a value-based primary care model.
