We are currently seeking a Primary Care Physician to work in a Federally Qualified Health Center (FQHC) and provide primary healthcare services to underserved populations within a community health center setting and coordinate care with other healthcare providers within the FQHC team.

Essential Functions:

  • Demonstrate the values and guiding principles of OhioGuidestone every day, performing with the highest standards of excellence, integrity, and ethics.
  • Patient Care: Provide direct, comprehensive primary care services, including the diagnosis, treatment, and management of acute and chronic medical conditions for patients across all life stages.
  • Preventive Care: Promote wellness and preventive care through patient education, health screenings, and lifestyle counseling.
  • Chronic Disease Management: Manage chronic conditions such as hypertension, diabetes, asthma, and others with a focus on improving patient outcomes and quality of life.
  • Care Coordination: Collaborate with interdisciplinary team members, including nurses, behavioral health specialists, case managers, and social workers, to ensure patients receive holistic and coordinated care.
  • Collaborate and support the development of Advanced Practice Providers.
  • Electronic Medical Records: Utilize the electronic health record (EHR) system to document patient encounters, update health information, and track patient outcomes.
  • Population Health: Participate in quality improvement initiatives and programs that aim to address health disparities and improve health outcomes for the patient population.
  • Patient Advocacy: Serve as a patient advocate, ensuring that care is equitable, culturally sensitive, and aligned with the patient’s health goals and social context.
  • Community Engagement: Participate in community outreach efforts, health fairs, and public health initiatives aimed at improving the overall health of the community served by the FQHC.
  • Maintain strong knowledge in field and support implementation of best practices.
  • Experience in the treatment and care of adults who have mental health and/or substance issues.
  • Exhibit positivity, flexibility, and a willingness to take on new responsibilities as requested or required.
  • Demonstrate positive leadership, promote an inclusive and equitable work environment, and present the Agency in the most positive light with all internal and external contacts.

Education & Experience:

  • Degree from an accredited school of Medicine or Osteopathy.
  • Board certified or eligibility in Family Medicine, Internal Medicine, and/or Pediatrics.
  • Valid, unrestricted medical license to practice medicine or osteopathy in the State of Ohio or ability to obtain one prior to starting.
  • DEA license and current BLS certification.
  • Demonstrated experience working in an FQHC, community health setting, or with underserved populations.

  • Ability to work effectively in a team-based care model.
  • Effective computer literacy skills, including Windows applications, Microsoft Office Suite, and electronic health records, etc.
  • Ability to customarily and regularly exercise discretion and good professional judgment.
  • Flexible; able to work days, evenings, and/or weekends, as desired to meet the needs of clients.
  • Strong organizational skills; able to handle multiple priorities.
  • Ability to maintain relevant, timely documentation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and effectively direct professional staff members.
  • Bilingual skills (preferred but not required, depending on patient population).
  • Possess a valid Ohio Driver’s License with a safe driving record and valid insurance.
  • Successfully meet pre-employment drug screen and all pre-employment background requirements.
