The Wisconsin Rapids VA Community-Based Outpatient Clinic seeks a Primary Care Physician to lead a Patient Aligned Care Team (PACT) delivering direct care to Veterans. The role emphasizes collaboration with RNs, LPNs, and clerical staff to ensure comprehensive, high-quality primary care.

Requirements include an MD or equivalent, a current full license to practice, residency training in a core specialty, and board eligibility or certification.

Qualifications

• U.S. citizenship requirement per VA policy.
• MD or equivalent degree from approved school.
• Current full and unrestricted medical license in the U.S.
• Completed residency in a core specialty with board eligibility or certification.
• English proficiency in speaking and writing.

Responsibilities

• Lead a Patient Aligned Care Team (PACT) delivering direct care to Veterans.
• Provide history, exam, diagnosis, and treatment initiation.
• Coordinate referrals, tests, and follow-up care with the team.
• Maintain up-to-date knowledge of clinical practices and policies.
• Collaborate with interdisciplinary partners to ensure continuity of care.
