Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.
Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

Pos i tion Summ a r y : The Primary Care Physician delivers high-quality, patient-centered comprehensive care to adults in an integrated healthcare clinic. This role emphasizes a holistic approach to wellness, coordination with behavioral health and specialty services, and a commitment to improving health outcomes within a collaborative, team-based environment.
Schedule: Mon-Fri, 8a-4:30p
Pos i tion R e s ponsibil i t i e s :
Provide high quality medical care in accordance with recognized standards of care.
Supervise mid-level providers and Medical Assistants ensuring the care team delivers high-quality care consistent with their professional responsibilities and clinical guidelines.
Adhere to clinic hours of operation, accepting consumers during scheduled hours in alignment with IHC’s established work schedule.
Conduct comprehensive patient assessments, including physical exams, health histories, and diagnostic tests (e.g., lab work); diagnose and treat a wide range of acute and chronic conditions.
Provide preventative care (e.g., vaccinations, screenings, health counseling), develop personalized treatment plans, prescribe medications, refer for imaging or to specialist as needed, and monitor patient responses to treatments.
Complete clinical documentation, including lab results, diagnostic imaging results refill requests, and other relevant patient records, in a timely manner that is consistent with agency standards.
Maintain accurate and timely medical records for all consumers examined.
Offer consulting services to other providers involved in consumer care to ensure proper integration of care.
Collaborate with MHC team to identify and close gaps in care for consumers in the IHC practice.
Provides health education and counseling as appropriate to consumers and their support system.
Offer on-going education and consultation with behavioral health providers, care managers, and nurses.
Assist in the development and implementation of clinical protocols related to primary care and preventative care.

E sse n t i a l S k i l l s a nd E x pe r i e n c e :
Unrestricted license to practice medicine in Tennessee.
Board eligibility or certification in Internal Medicine or Family Practice.
Two years of practice experience in an adult primary care setting preferred.
Candidate must complete and pass credentialing process.
Candidate must obtain credentials from all relevant government agencies, health plans or facilities.

Physical demands:

MHC was founded in Nashville, Tennessee, in response to the need for assertive community-based treatment for individuals who have required frequent inpatient psychiatric hospitalization. Since 1993, MHC has been passionately helping adults with severe mental illness to live quality lives in the community with our state-of-the-art treatment and support. In 1999, MHC recognized the need to support the whole family and expanded services to include children and youth.
We established our non-profit agency by merging three community organizations: Urban Case Management of Meharry Community Health Center, Continuous Treatment Team of Council of Community Services, and Mobile Crisis Team of the Crisis Intervention Center.
This fusion resulted in an innovative, comprehensive, and multidisciplinary approach that addresses the specialized needs of individuals living with severe behavioral health conditions and challenges related to poverty. MHC remains the only organization in the state focused solely on this population.
