• Medical degree (MD or DO) from an accredited institution.
• Completion of residency in Internal Medicine or Family Medicine.
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.

Responsibilities

• Manage a small patient panel focused on seniors with multiple chronic conditions.
• Conduct comprehensive outpatient primary care visits.
• Develop and maintain personalized care plans to improve patient outcomes.
• Collaborate with a support team including MA, scheduler, and referral specialist.
• Engage in value-based care practices emphasizing prevention and chronic disease management.
• Participate in clinic hours Monday through Friday with no night or weekend responsibilities.
• Allocate administrative time for documentation and care coordination.

Primary Care Physician (MD/DO) – Summary

• Full-time role focused on outpatient primary care for seniors
• Work in a physician-led, mission-driven, value-based care organization

Duties & Responsibilities

• Manage a small patient panel (maximum 450 patients), primarily seniors with multiple chronic conditions
• Conduct comprehensive outpatient primary care visits (average 18 patients per day)
• Develop and maintain personalized care plans to improve patient outcomes
• Collaborate with a dedicated support team, including MA, scheduler, and referral specialist
• Engage in value-based care practices emphasizing prevention, wellness, and chronic disease management
• Participate in clinic hours Monday through Friday, with no night or weekend responsibilities
• Allocate administrative time within the work schedule for documentation and care coordination
• Refer patients to in-house consulting specialists as needed (e.g., cardiology, podiatry)
• Advise on holistic health services available within the clinic (acupuncture, Tai Chi, yoga, nutrition)
