Job summary

HealthyU Clinics is seeking a Primary Care Provider to lead a care team and deliver high-quality, patient-centered primary care to an assigned panel. The Provider serves as a clinical leader, offering medical oversight, expertise, and guidance to support coordinated care and positive patient outcomes.

Responsibilities include health assessments, diagnosing and treating conditions, preventive care, and maintaining accurate records in compliance with standards.

Qualifications

• Completion of an accredited residency program in Family Medicine or Internal Medicine.
• Active, unrestricted medical license in the state where the physician will practice.
• Current Basic Life Support (BLS) certification.
• Current, unrestricted DEA registration.
• Minimum of two years of clinical practice experience.
• Demonstrated experience providing comprehensive, longitudinal primary care.
• Knowledge of managed healthcare systems, including quality assurance and disease management programs.
• Ability to work effectively within a multidisciplinary team.
• Excellent communication and interpersonal skills.
• Two professional references.

Responsibilities

• Provide comprehensive, patient-centered primary care consistent with value-based care model.
• Conduct health assessments; diagnose and treat acute and chronic conditions; develop treatment plans.
• Provide preventive care, education, and counseling to promote wellness.
• Collaborate with physicians, APPs, nurses, and other professionals to ensure coordinated care.
• Perform minor office-based procedures within licensure and privileges.
• Maintain accurate patient records and comply with regulatory standards.
• Participate in quality improvement initiatives and monitor performance metrics.
• Support adoption of new healthcare technologies and programs.
