Job summary

HealthyU Clinics is seeking a Primary Care Provider to lead a care team and deliver high-quality, patient-centered primary care to an assigned panel. The Provider serves as a clinical leader, offering medical oversight, expertise, and guidance to support coordinated care and positive patient outcomes.

Responsibilities include health assessments, diagnosing and treating conditions, preventive care, and maintaining accurate records in compliance with standards.

Qualifications

  • Completion of an accredited residency program in Family Medicine or Internal Medicine.
  • Active, unrestricted medical license in the state where the physician will practice.
  • Current Basic Life Support (BLS) certification.
  • Current, unrestricted DEA registration.
  • Minimum of two years of clinical practice experience.
  • Demonstrated experience providing comprehensive, longitudinal primary care.
  • Knowledge of managed healthcare systems, including quality assurance and disease management programs.
  • Ability to work effectively within a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Two professional references.

Description

As a leader of the care team, the Primary Care Provider delivers high-quality, comprehensive, and patient-centered primary care to an assigned panel of patients. The Provider serves as a clinical leader, offering medical oversight, expertise, and guidance to support coordinated care, positive patient outcomes, and the delivery of accessible, high-value healthcare to the communities served. Key responsibilities include:

  • Provide comprehensive, patient-centered primary care consistent with HealthyU Clinics' value-based care model.
  • Conduct comprehensive health assessments; diagnose and treat acute and chronic conditions; and develop individualized treatment plans.
  • Provide preventive care, health education, and counseling to promote wellness and support positive health outcomes.
  • Collaborate effectively with physicians, advanced practice providers, nurses, and other healthcare professionals to ensure coordinated, comprehensive, and continuous patient care.
  • Perform minor office-based procedures within the scope of licensure, training, and clinical privileges.
  • Respond appropriately to medical emergencies and provide timely intervention within the scope of clinical practice.
  • Maintain accurate, timely, and complete patient medical records and documentation in accordance with clinical, legal, regulatory, and organizational requirements.
  • Actively participate in quality improvement initiatives and monitor practice performance metrics to support continuous improvement and high-quality patient care.
  • Adhere to all privacy, compliance, regulatory, and quality standards, as well as HealthyU Clinics' policies, procedures, protocols, and Code of Conduct.
  • Support the implementation and adoption of new healthcare technologies, products, services, and programs that advance HealthyU Clinics' mission and strengthen its role in community health and wellness.
  • Maintain current knowledge of medical advancements, evidence-based practices, and applicable clinical standards through continuing medical education, conferences, seminars, and other professional development activities.
  • Perform other duties and responsibilities as assigned.
Requirements
  • Completion of an accredited residency program in Family Medicine or Internal Medicine.

  • Current Basic Life Support (BLS) certification.
  • Current, unrestricted DEA registration.
  • Minimum of two years of clinical practice experience.

  • Knowledge of and experience with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programs.
  • Ability to work effectively and collaboratively within a multidisciplinary healthcare team.
  • Excellent communication, interpersonal, and relationship-building skills.
  • Self-motivated, results-oriented, and committed to ongoing professional development and clinical excellence.
  • Two professional references.
