Job summary

Health Center Inc., Harlem location, seeks a Primary Care Physician to provide comprehensive care to a panel of patients with a collaborative care team. The role offers a stable schedule, no weekends and no overnight shifts.

Required: active NYS license, DEA, BCLS, infection control certificate, and recent prescriber education in pain management/palliative care/addiction. Benefits include competitive salary and employer-sponsored plans.

Qualifications

• Active New York State physician license required.
• DEA certificate required for prescribing controlled substances.
• BCLS certification required for basic life support.
• Infection control certificate required.
• Completed NYS DOH mandated prescriber education in pain management, palliative care or addiction within past 3 years.

Responsibilities

• Provide comprehensive primary care to a designated panel of patients in collaboration with the care team.
• Ensure complete and timely documentation in the electronic medical record.
• Collaborate with other health professionals to coordinate treatment plans.
• Communicate results and follow-up steps to patients.
• Lead the care team in patient education.
• Perform other duties as assigned.
