Physician Alliance of Connecticut (PACT, LLC) seeks a physician to serve in Skilled Nursing Facilities in Milford, CT. The role offers a full-time, on-site schedule with 40 hours per week, focusing on primary care within a multi-specialty group.

Qualifications

• Medical Doctor with 3 to 5 years of direct patient experience including residency.
• Medical License, Board certification, Federal DEA certification
• Need to be Board eligible or Board Certified

Responsibilities

• Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
• Analyze patients medical history, medication allergies, physical condition and examination to determine best care
• Diagnose medical condition
• Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
• Directs and coordinates the patient care activities of nursing and support staff as required.
