Job summary

Qualifications

• Unrestricted medical licensure in Indiana required.
• Board certification or eligibility in Family Medicine, Internal Medicine or Med-Peds required.
• Experience in primary care with a patient-centered approach preferred.

Responsibilities

• Provide comprehensive primary care to a diverse patient panel with the support and resources needed.
• Collaborate with a multidisciplinary team including care coordinators, behavioral health, pharmacy, and other care providers.
• Perform in-office procedures such as simple skin surgery, fracture care, joint injections, maternity care, IUDs, etc. (if desired).
• Participate in quality improvement and population health initiatives.
• Educate and empower patients to take ownership of their health.

Education

Board certified or board eligible in Family Medicine, Internal Medicine or Med-Peds
