Qualifications

• Unrestricted medical licensure in Indiana.
• Board certified or board eligible in Family Medicine, Internal Medicine or Med-Peds.
• Active and unrestricted DEA license.

Responsibilities

• Provide comprehensive primary care to a diverse patient panel with the support to practice at the top of your license.
• Collaborate with a multidisciplinary team including care coordinators, behavioral health, pharmacy, and other care providers.
• Participate in quality improvement and population health initiatives.
