Primary Care Physician Assistant/Primary Care Family Nurse Practitioner

CLASSIFICATION/STATUS: Exempt, Licensed, Full Time

Supervisory Responsibilities: None

Pay Range: Band G w/o panel ($105,720-$134,000/year),Band H w/ panel ($120,000-$146,000/year)

As part of the dynamic Primary Care team at CRCH, the Primary Care Physician Assistant/Primary Care Family Nurse Practitioner provides direct care and education to Health Center patients and ensures continuity and quality of care. The Physician Associate/Nurse Practitioner will have a solid understanding of patient needs and will work collaboratively with a multidisciplinary team to co-manage primary care patients.

YOUR RESPONSIBILITIES

  1. Provide direct patient care, counseling, and teaching to the primary care population in the ambulatory setting. Care for both acute and chronic conditions for new and established patients.
  2. Perform complete histories and physical examinations.
  3. Order, interpret, and evaluate appropriate laboratory and diagnostic tests.
  4. Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
  5. Order medications and write prescriptions according to organizational and regulatory policies and procedures, including agreement with collaborating physician as required.
  6. Consistently follow through on high quality and timely documentation.
  7. Collaborate and consult with a multidisciplinary team of providers and other staff, including supervising physician, as appropriate.
  8. May perform procedures as agreed upon with the Medical Director.
  9. Practice cost-effective medicine efficiently, maximizing available resources.
  10. Demonstrate expert clinical judgment skills to function in an independent role.
  11. The role will consist of 32 clinical hours and 8 admin hours per week.
  12. Perform other clinical duties as assigned by Lead Physician Assistant, or designee.

Organizational Citizenship:

  1. Participate in community outreach projects and Center meetings as requested.
  2. Participate in Quality Assurance activities as directed.
  3. Maintain a learning mindset with active involvement in individual continuing education and participation in all required organizational or clinical training.
  4. Perform other related duties as required by the Medical Director.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

PA Qualifications:

  1. Completion of a graduate-level (Master’s degree) physician assistant program accredited by the Accreditation and Review Committee for Physician Assistants (ARC-PA), which qualifies the candidate to take the NCCPA certifying exam, required.
  2. Certification by the National Commission on Certification of Physician Assistants (NCCPA) must be obtained prior to the first day of employment.
  3. Must be eligible for or already hold an active Massachusetts license as a Physician Assistant.
  4. Must maintain a current Massachusetts Controlled Substance Registration (MCSR) and DEA registration.
  5. Must complete CME as required for maintenance of NCCPA certification and MA licensure. Must re-certify as a PA-C on the interval set by NCCPA.
  6. Must apply for and be granted privileges at affiliated hospital(s).
  7. Understanding of ambulatory family practice health care program administration and clinical operations preferred.
  8. Experience with Patient Centered Medical Home model and concepts preferred.
  9. Knowledge of and experience with EMR and Microsoft Office systems required, familiarity with Epic desired.
  10. Must be able to work out of either our Brighton or Waltham locations as needed.
  11. Fluency in Spanish and/or Portuguese a plus.
  12. Must have excellent communication skills, particularly with people from diverse cultures whose primary language is not English, with the ability to understand the community, population, and patients we serve.
  13. Strong commitment to promoting diversity, equity, and inclusion and reducing inequities.
  14. Participate in continuing medical education activities as required for licensure, and take exams as required by specialty board to maintain board certification.
  15. Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.

NP Qualifications:

  1. Must be a graduate of Master's of Science in Nurse Practitioner program or have completed a Certified Nurse Practitioner program.
  2. Must have Massachusetts certification in Expanded Role from Board of Registration in Nursing.
  3. Must have Massachusetts Nursing License.
  4. Must have Nurse Practitioner Certification.
  5. Must maintain a current Massachusetts Controlled Substance Registration (MCSR) and DEA registration.
  6. PCP candidates must be eligible for Full Practice Authority in the state of Massachusetts.
  7. Must complete continuing education as required to maintain Nurse Practitioner licensure.
  8. Must apply for and be granted privileges at affiliated hospital(s).
  9. Understanding of ambulatory family practice health care program administration and clinical operations preferred.
  10. Experience with Patient Centered Medical Home model and concepts preferred.
