Job summary

Responsibilities

• Provide treatment to individuals with mental or physical injury or illness, dysfunctional behavior and/or developmental disability.
• Maintain continuity of health care and guide individuals and their interdisciplinary habilitative treatment team members in formulating and implementing comprehensive individual habilitative care and treatment program plans.

Education
