Primary Care Physician Responsibilities:

• Provide comprehensive care for patients with acute and chronic illnesses, managing a panel of up to 450 patients.
• Build trusting relationships to influence patient outcomes positively.
• Act as a health coach, helping patients set and achieve health goals.
• Simplify and prioritize care plans for effective behavior change.
• Collaborate with care teams and specialists, and participate in quality and risk management programs.

Qualifications:

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• Current or eligible for State Medical License.
• Board certification or eligibility in relevant specialty.
• Current DEA registration and BLS certification.
