Qualifications

  • TB screening completed for patient-facing roles.
  • Ability to work independently and manage complex care.

Job description

We are seeking a dedicated Physician to provide comprehensive outpatient medical care focused on continuity of care, preventive medicine, chronic disease management, and patient wellness. This role is responsible for evaluating, diagnosing, and treating a variety of health conditions while building long-term patient relationships and supporting positive healthcare outcomes.

Responsibilities
  • Provide comprehensive outpatient medical care and treatment
  • Diagnose and manage acute and chronic medical conditions
  • Conduct patient evaluations and maintain detailed medical histories and records
  • Develop individualized treatment plans and monitor patient progress
  • Focus on preventive care, wellness, and disease prevention initiatives
  • Educate patients on treatment options, health conditions, and lifestyle modifications
  • Coordinate care and refer patients to specialists when appropriate
  • Review diagnostic test results and incorporate findings into care plans
  • Collaborate with clinical staff and healthcare teams to ensure quality patient care
  • Support initiatives that improve patient outcomes and enhance the patient experience
Required Qualifications
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
  • Active and unrestricted medical license in the applicable state of practice
  • Board Certification in:
  • Family Medicine, or
  • Internal Medicine, or
  • Geriatric Medicine
  • Minimum of 5 years of clinical experience
  • Strong knowledge of outpatient and primary care medicine
  • Excellent clinical judgment, communication, and interpersonal skills
  • Ability to work independently and manage complex patient care situations
  • Successful completion of required Tuberculosis (TB) screening for patient-facing healthcare roles
  • Passion for improving patient experiences and delivering high-quality healthcare
Preferred Qualifications
  • Internal Medicine specialty training
  • Experience in preventive care, value-based care, or population health management
  • Experience working with diverse patient populations
  • Strong collaboration and leadership skills within multidisciplinary healthcare teams
