Job summary

Apex Bridge Talent Group is seeking a Primary Care Physician to join a mission-driven team delivering comprehensive, community-based care to seniors in California. The role emphasizes home visits, integrated care, and collaborative planning with the interdisciplinary team.

The physician will conduct in-person assessments, develop personalized care plans, and support discharge planning while aligning with value-based care.

Qualifications

• Completion of an ACGME-accredited residency.
• MD or DO degree required; board certification preferred.
• Active state physician license without restrictions; CDS and DEA registrations required.

Responsibilities

• Perform comprehensive in-person assessments with the IDT to craft patient-centered care plans.
• Coordinate discharge planning for acute, short-term, and long-term placements.
• Maintain focus on comprehensive care with regular home visits and no hospital rounds.

Job description

• Our value based THE CLIENT program allows our Primary Care Physicians to provide truly comprehensive, holistic care, so you can focus on what really matters: your patients.
• Regular hours of 8am-5pm, Monday through Friday, including home visits.
• Rotating on-call schedule - a few nights or weekends per month, available by phone only.
• CME reimbursement and malpractice/tail coverage.

Essential job duties include:

• Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds focus on comprehensive care, seeing an average of 8 patients a day

Job requirements include:

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Master's or doctorate Degree
• Board Certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.
• We are seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
• Must be eligible for enrolment to provide services to Medicare and Medi-Cal participants.
• Board certification preferred.
