Authentic Group of Companies is seeking a Primary Care Physician in Stockton, CA to deliver comprehensive, patient-centered care to older adults within a value-based model. You’ll work with an interdisciplinary team, perform home visits, and manage individualized care plans aimed at improving seniors’ health outcomes.

Formación

• MD or DO degree
• Completion of an ACGME-accredited residency
• DEA registration and applicable California CDS certification
• Background in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
• Eligible to enroll as a provider for Medicare and Medi-Cal participants
• Must be able to obtain CPR and First Aid certification within 180 days of hire if not already certified

Primary Care Physician | Stockton, California

Make a meaningful difference in the lives of older adults through comprehensive, community-based healthcare.

We are seeking a Primary Care Physician to join a mission-driven interdisciplinary team providing personalized, coordinated care to seniors through a value-based care model.

Position Highlights:

• Approximately 8 patients per day
• Home visits included
• Rotating phone-only on-call coverage a few nights or weekends per month
