Job description

Responsibilities

The Primary Care Physician (PCP) demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for a panel of up to 450 patients. The PCP serves as a health coach, helping patients set short‑ and long‑term health goals and working with them toward those goals. The PCP simplifies and prioritizes care to make behavior change actionable for patients and leads care teams toward performance goals.

  • Accountability for outcomes: Strong clinical care and cost‑effectiveness for each patient.
  • Coaching for health: Act as a health coach to help patients set and achieve health goals.
  • Simplifying for action: Simplify and prioritize care to facilitate behavior change.

Performance metrics include patient admissions per thousand, use of 18‑21 appointment slots per day, CGCAHPS, clinical gap closures, and medical cost measures. Each PCP sets goals for these metrics and works toward them with center and market leadership.

Essential Job Duties & Responsibilities
  • Independently provide care for patients with acute and chronic illnesses in older adults and take full accountability for patient care and outcomes.
  • Assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for acute and complex chronic patients.
  • Lead care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office‑visiting patients.
  • Engage with health‑care teams (e.g., hospitalists, transitional care, case managers) for patients unable to visit the office.
  • Collaborate with specialists; remain responsible for detailed patient care while seeking consultation as needed.
  • Cover for other providers and assist in center management, including recruitment of patients and additional providers.
  • Perform additional duties as assigned and modified at manager’s discretion.

  • Availability and accessibility: Provide walk‑in visits and respond to phone calls and messages in a timely manner.

  • Evidence‑based medicine: Stay updated on evidence‑based treatments, including lifestyle and nutrition interventions.
  • Physician leadership: Continuously develop leadership skills to benefit patients, team, center, and company.
  • Quality focus: Work with care teams to achieve high‑quality outcomes and quality measures.
  • Influence: Competently and compassionately influence patients, teams, and yourself to achieve the best outcomes.
  • Self‑care: Maintain personal health in physical, mental, social, and spiritual domains.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software.
  • Willingness to travel locally and occasionally nationally for training and recruiting.
  • Fluency in English; additional languages may be required depending on the center.
Education and Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required. The candidate must obtain or already hold a current, active state medical license. Board certification in a relevant specialty is preferred; board eligibility is required. Board‑certified PCPs must maintain certification through MOC, CME, or recertification exams as required. A current DEA number for schedule II‑V controlled substances is required. Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross is required within the first 90 days of employment.

Pay Range

Salary: $221,141 – $315,915 (base hourly rate or annual full‑time salary). Compensation may vary based on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
