ChenMed LLC is looking for a Primary Care Physician (PCP) in Memphis, TN, to provide independent medical care to patients with chronic and acute illnesses. Responsibilities include patient assessment, diagnosis, treatment, and care coordination with a dedicated team. The PCP will ensure quality outcomes and maintain comprehensive patient management, while adapting to flexible working hours as needed.
The ideal candidate will hold an MD or DO with a state medical license and be proficient in relevant specialties. Comprehensive benefits and competitive salaries ranging from $221,141 to $315,915 are offered along with career development opportunities.
The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult population, taking full accountability for patient care and outcomes. The PCP will seek consultation from specialists when needed while remaining involved in the detailed care of the patient. It is expected that the PCP will engage with the hospitalist whenever the patient is hospitalized, regardless of the hospitalist’s affiliation with ChenMed. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients unable to attend the office—whether in hospital, SNF, LTC, or homebound—the PCP will engage with the transitional care team, case managers, acute and transitional‑care physicians, and other resources available in the market. The PCP also plays an active role in center management, covers for other providers when needed, and assists in recruiting patients and additional providers. The PCP supervises, collaborates with, participates with, or functions within a collaborative agreement with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions while employed by the company. The PCP performs other duties as assigned and modified at the manager’s discretion.
$221,141 – $315,915. The posted range represents the base hourly rate or base annual full‑time salary; final compensation may vary based on experience, education, location, and other factors. Additional bonuses or commissions may be eligible.
