Responsibilities

• Patient Care: Provide high-quality, patient-centered care, including preventive services, diagnosis, and management of acute and chronic conditions.• Coordination of Services: Serve as a “health manager” by coordinating patient care, guiding diagnostic procedures, and facilitating referrals to specialists when necessary. • Health Education: Counsel and educate patients on safe health behaviors, self-care skills, treatment options, and preventive healthcare services. • Collaboration: Work collaboratively with a multidisciplinary team to ensure comprehensive and continuous care for patients.
