The Physician provides comprehensive outpatient care focused on preventive medicine, chronic disease management, continuity of care, and improving overall patient health outcomes. This role emphasizes building meaningful patient relationships while delivering high-quality, value-based care in a collaborative, team-oriented environment. You will report directly to the Area Medical Director. This practice will be onsite at our Conviva Pompano East clinic.

Your Day in the Life of a Conviva Physician

• Monday through Friday, full-time 8:00 a.m. - 5:00 p.m.
• Versatile care team with an established patient panel
• See approximately 12-15 patients per day, allowing for a more personalized approach to care
• Longer appointment times, 40-minute for new patients
• Spend your time focused on direct patient care, supported by a dedicated clinical and operational team
• Work within a value-based care model designed to improve patient outcomes and enhance the provider experience

Required Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active, unrestricted medical license in the state of practice (or willingness to obtain)
• Current, unrestricted DEA license

Preferred Qualifications

• Two or more years of clinical practice experience
• Experience managing Medicare Advantage patient populations
• Knowledge of Medicare regulations and coverage guidelines
• Familiarity with HEDIS quality measures and value-based care initiatives
• Bilingual Haitian Creole and English perfered

This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
