Job summary

Conviva Senior Primary Care seeks a Physician to provide outpatient care with a focus on continuity, preventive medicine, and timely referrals. You will maintain patient histories and work within an integrated care team at an outpatient clinic, reporting to the Associate Medical Director.

The role requires MD/DO, state licensure, and board certification in Family, Internal, or Geriatric Medicine, with 40 hours per week and participation in TB screening as part of Humana/Senior Bridge programs.

Qualifications

• MD/DO with state licensure and board certification in Family, Internal or Geriatric Medicine.
• Outpatient-focused practice with emphasis on preventive care and ongoing health maintenance.
• Licensure requirements of the state of jurisdiction.
• Able to work within Humana/Senior Bridge programs including TB screening.

Responsibilities

• Provide outpatient care focusing on continuity of care and disease prevention.
• Maintain patient history and medical records; refer to specialists as needed.
• Collaborate with the Associate Medical Director and clinic team on strategy.
• Work in an outpatient clinic setting with some limited on-call coverage by phone.

The Physician serves as a health-care professional and capable of handling a variety of health-related problems. The Physician work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.

The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy. Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction. Exercises considerable latitude in determining objectives and approaches to assignments. You will report to the Associate Medical Director and work in an outpatient clinic setting.

• Bachelor's degree
• 5 or more years of technical experience
• Licensure requirements of the state of jurisdiction
• Graduate of accredited MD or DO program of accredited university
• Prefer Internal Medicine specialty
• Board Certification in Family Medicine, Internal Medicine or Geriatric Medicine

This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Must be willing to participate in limited on-call coverage by phone

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
