Qualifications

• Doctor of Medicine or Doctor of Osteopathic Medicine degree.
• Completion of an accredited residency program.
• Board certified or board eligible in Family Medicine, Internal Medicine, or another primary care specialty.
• Active Florida medical license or eligibility to obtain Florida licensure.
• Active DEA registration or ability to obtain.
• Experience caring for Medicare/geriatric populations preferred.

Responsibilities

• Provide comprehensive outpatient care to a Medicare‑aged population.
• Develop long‑term patient relationships and preventive plans.
• Perform histories, exams, assessments, and diagnoses.
• Manage chronic conditions and order/interpret tests and imaging.
• Prescribe medications and monitor treatments.
• Coordinate care with specialists and healthcare team.
• Maintain accurate EMR documentation.
• Support patient education and screenings.

We are seeking a full-time Primary Care Physician to join our Orange Park practice. Our physicians provide comprehensive, patient-centered care to a primarily Medicare-aged population, with a strong focus on preventive medicine, chronic disease management, and continuity of care.

This is an excellent opportunity for a physician who enjoys developing long-term relationships with patients while maintaining the stability of a Monday–Friday schedule with no nights, weekends, or call.

What You’ll Do

As a Primary Care Physician, you’ll provide comprehensive outpatient care while developing long-term relationships with your patients.

Responsibilities Include

• Perform comprehensive histories, physical examinations, assessments, and diagnoses
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment and preventive care plans
• Manage chronic conditions commonly seen in a Medicare-aged population
• Order and interpret laboratory testing, diagnostic studies, and imaging
• Prescribe medications and monitor ongoing treatment
• Provide preventive care, health screenings, and patient education
• Coordinate care with specialists and other members of the healthcare team
• Review and manage patient care needs between visits as appropriate
• Maintain accurate and timely documentation in the electronic medical record

What We’re Looking For

• Doctor of Medicine or Doctor of Osteopathic Medicine degree
• Completion of an accredited residency program
• Board certified or board eligible in Family Medicine, Internal Medicine, or another appropriate primary care specialty
• Active and unrestricted Florida medical license or eligibility to obtain Florida licensure
• Active DEA registration or ability to obtain
