Job description

The staff physician shall work collaboratively with the MCHC health center team(s) to develop treatment plans and provide primary health care services and education to patients based upon their medical and psychosocial needs. Provision of services shall be in accordance with all applicable Federal and State laws, rules and regulations and in compliance with patient's Bill of Rights, based upon evidence-based care standards.

  1. Responsibilities:
  2. Provide quality medical care to patients in accordance with accepted standards and policies.
  3. Review all medical histories upon initial visit and maintain accurate documentation within the electronic medical record system.
  4. Coordinate medical care for patients on their panel.
  5. Triages walk-in patient needs and referrals, and coordinates medical care accordingly.
  6. Consult with the Chief Medical Officer for clinical and administrative supervision regularly.
  7. Collaborate with the healthcare team to ensure proper patient care and coordination.
  8. Participate in daily huddles and practice team meetings as a member of the Patient Care Team.
  9. Assist in patient care activities related to Patient Centered Medical Home and other programs, including ongoing care planning for high-risk groups.
  10. Participate in ongoing care planning and review.
  11. Provide onsite medical interventions when necessary and refer patients to additional services within the MCHC Network or outside facilities for services not available at the center.
  12. Be sensitive to cultural diversity and promote this among staff and patients.
  13. Undertake tasks assigned by the Chief Medical Officer and/or Chief Operating Officer.
  14. Ensure prompt follow-up on patients with abnormal test results or physical findings.
Requirements:
  • Hold a Medical Degree (MD) from a recognized medical school.
  • Be currently licensed and registered as a physician with the New York State Education Department.
  • Be Board Certified or Board Eligible in a recognized medical specialty.
  • Be BLS Certified.

Pay Range:
