Full-Time, Monday to Friday, 8:00 AM - 5:00 PM

Role Description

• Provide equitable, effective value-based healthcare to Medicare patients in neighborhood clinics.
• Operate within a large care team, focusing on quality over volume.
• Rewarded for outcomes, low hospital admissions, preventive care, and patient experience.

Responsibilities

• Assess, diagnose, and oversee primary care for patients.
• Prescribe medications and maintain electronic health records.
• Collaborate with leadership to meet quality goals.
• Manage center care team with Practice Managers.

Qualifications

• MD or DO degree; Board certification or eligibility in Internal or Family Medicine.
• Active, unrestricted State License and DEA license.
• US work authorization and embodying “Oaky” values.

Preferred Qualifications

• Geriatrics fellowship, experience with older adults, value-based care, outpatient settings, bilingual skills.
