Job description

Primary Care Physician (PCP)

Responsibilities

• Independently provide comprehensive care for older adult patients with acute and chronic illnesses.
• Maintain full accountability for patient care and outcomes, seek specialist consultation as needed.
• Engage with hospitalists for admitted patients and coordinate care across inpatient, SNF, LTC, and home settings.
• Lead a care team consisting of a medical assistant, care facilitator, and care coordinator for office‑based patients.
• Participate in transitional care teams and collaborate with case managers, acute and transitional‑care physicians, and other resources.
• Assist in center management, cover for other providers, and help recruit patients and additional providers.
• Supervise, collaborate, or participate with an APP, remaining accountable for the APP’s actions while employed.
• Perform other duties as assigned and modified by manager.

Education & Experience

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• State medical license in the state of work; eligible for or board‑certified in chosen specialty; must maintain certification via MOC, CME, or exam re‑certification.
• Current DEA registration for schedule II–V controlled substances.
• Basic Life Support (BLS) certification from American Heart Association or American Red Cross within first 90 days of employment.
• Board eligibility required; board certification preferred.

Compensation

Pay range: $221,141–$315,915 (base hourly rate or full‑time salary, dependent on experience, education, location, etc.). Potential bonuses or commissions may apply.
