Job description

ESSENTIAL JOB DUTIES/RESPONSIBILITIES
  • Independently assess acute and non‑acute clinical problems, perform and document physical assessments and patient histories, analyze trends in patient conditions, and develop, document, and implement patient management plans in response to the data obtained. This also includes assisting in the development of the nursing plan of care and providing appropriate patient/family/significant‑other counseling and education.
  • Plan patient care based on in‑depth knowledge of the specific patient population and/or protocol, anticipate and identify physiological and/or psychological problems commonly encountered, and consider the patient’s cultural background, level of understanding, personality and support systems. Serve as patient advocate.
  • Write admission, transfer, and discharge orders; order and interpret appropriate laboratory and diagnostic studies; order appropriate medication and treatments; refer patients for consultation when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc.); document through in‑depth progress notes and summaries.
  • May perform invasive procedures independently upon completion of documented competency. Participate in patient care rounds and conferences.
  • Communicate patient management strategies to members of the patient care team and collaborate with multidisciplinary team members to ensure strategies are successful.
  • Recognize situations requiring immediate physician attention and initiate life‑saving procedures when necessary. Use advanced communication skills to problem‑solve complex situations and improve processes and service to patients.
  • Collaborate with multidisciplinary team to analyze and evaluate current health‑care systems to identify and implement new practice patterns. Participate in outside activities that enhance personal and professional growth. Initiate arrangements and write orders for SNF discharges; work collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as medically appropriate; introduce self to patient/family and explain nurse practitioner role; facilitate patient/family conferences; enhance collaborative relationships to maximize informed decisions; facilitate discharge to appropriate level of care; prescribe medication based on state practice; perform duties as assigned and modified at manager’s discretion.

EDUCATION AND EXPERIENCE CANDIDATE REQUIREMENTS
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty required.
  • Must be able to obtain a state medical license or already hold a current, active state medical license in the state(s) where the position will be performed.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty preferred; board eligibility required. After certification, maintain board status through MOC, CME, or retaking exams as required.
  • Must have a current DEA number for schedule II–V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment.

$221,141 – $315,915
