Job summary

InnovaCare Health seeks a physician to provide primary patient care at our Florida clinics, applying medical principles to improve patient outcomes and coordinate with a multidisciplinary team.

The role supports chronic disease management, preventive care, and patient education within a value-based model. Applicants must hold MD/DO and be board-certified or board-eligible, with an active Florida license or eligibility.

Qualifications

• Must hold an MD or DO and be board-certified or board-eligible in Internal Medicine, Family Medicine and/or Geriatric Medicine.
• Current, unrestricted Florida medical license or eligibility to obtain one.
• Experience in value-based healthcare is a plus.

Responsibilities

• Provide primary patient care applying and modifying medical principles.
• Coordinate with care teams to manage chronic disease and preventive care.
• Engage in community health education and outreach as part of the mission.
