Role Overview

The Primary Care Physician (PCP) demonstrates accountability for outcomes, providing strong clinical care and managing cost-effectiveness for up to 450 patients. They act as health coaches, helping patients set and achieve health goals, and simplify care plans to promote actionable behavior change.

The PCP works independently and collaboratively, engaging with care teams, specialists, and hospitalists to ensure comprehensive patient care. They are responsible for assessment, diagnosis, treatment, education, and care coordination for patients with acute and chronic illnesses, and lead their care teams effectively.

Essential Duties

1. Provide care for patients with acute and chronic illnesses, maintaining full accountability for outcomes.
2. Engage with hospitalists and care teams for hospitalized patients.
3. Manage assessment, diagnosis, treatment, and health promotion activities.
4. Lead care teams and coordinate care for patients unable to visit the office.
5. Participate in practice management, recruitment, and compliance activities.

Qualifications
