Job summary

GQR Healthcare is offering a Primary Care opportunity in Connecticut within a true team-based care model that supports physicians to provide high-quality outpatient medicine. You will see about 18 patients per day with 15- and 30-minute appointments, backed by a MA and RN, and daily morning huddles to coordinate patient care.

The M-F 8am-5pm schedule includes one late evening per week, with EPIC used for care coordination.

• Work with a MA and RN daily to coordinate patient care.
• Participate in daily morning huddles to coordinate patient care.
• Use EPIC for documentation.
• Assist patients after hospital discharge as needed through coordinated care.

Starting compensation is $250K with the opportunity for increased compensation based on community health experience and/or Spanish-speaking ability. Excellent benefits to include insurance, PTO, CE allowance, reimbursement for state licensure, professional dues & DEA fees.
