Position Details

Location: 2240 East 53rd St, Suite B-1, Indianapolis, IN 46220

Schedule: Full-Time, Monday through Friday, 8:00 AM - 5:00 PM

Role Overview

The Primary Care Physician at Oak Street Health provides equitable, effective, value-based healthcare to Medicare patients in neighborhood clinics. Operate within a collaborative team supported by large care teams, focusing on quality care, preventive medicine, and positive patient outcomes. Compensation includes competitive bonuses based on outcomes.

Responsibilities

• Assess and diagnose patients
• Oversee primary care services
• Prescribe medications and treatments
• Maintain electronic health records
• Collaborate with leadership to meet quality goals
• Manage care teams with Practice Managers
• Perform other duties as assigned

Qualifications

• MD or DO degree
• Board certification or eligibility in Internal Medicine or Family Medicine
• Active, unrestricted license and DEA registration
• US work authorization
• Motivated to serve older adult populations

Preferred Skills

• Geriatrics fellowship
• Experience with Medicare and value-based care
• Team collaboration skills
• Bilingual abilities (if applicable)
