Job summary

Care Alliance Health Center in Cleveland, OH is seeking a compassionate Physician Assistant to join our integrated primary care team. You will provide comprehensive assessment, diagnosis, treatment, preventive care, and chronic disease management in outpatient settings.

Collaborate with an interdisciplinary team to improve outcomes for diverse communities. The role requires ARC-PA accredited PA program degree, NCCPA, Ohio license, prescriptive authority, BLS, and strong communication.

Responsibilities

• Conduct comprehensive and focused patient assessments and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Develop and implement evidence-based treatment plans.
• Order and interpret laboratory testing, diagnostic imaging, and other appropriate studies.
• Prescribe medications within applicable scope and prescriptive authority.
• Provide preventive care, screenings, immunizations, and chronic disease management.
• Educate patients and families about diagnoses, treatment options, medications, prevention, and healthy lifestyle behaviors.
• Identify care gaps and support patients in achieving their health goals.
• Coordinate referrals and collaborate with physicians, nurses, behavioral health providers, pharmacists, care coordinators, Community Health Workers, and other members of the care team.
• Maintain timely and accurate clinical documentation and appropriate patient follow-up.
